Primary care networks are groups of GP practices working together to make sure their services meet the needs of local people. They are a key part of the NHS Long Term Plan.
Across West Lancashire our primary care networks work closely with other health and care services within our three ‘neighbourhoods’ (as detailed above) to provide more personalised and joined up care. They will become the bedrock of health and care services in the future.
In total we have 15 GP practices working within the neighbourhoods. Through neighbourhoods you will see more joint working and we will aim to provide as much support as we can in a community setting.
